Students under 19 are required to have parental consent on their application form.
Parent/Guardian, please read and initial the section below:
- I verify that all information on this enrollment form is accurate and complete and that the legal names and birth date have been entered exactly as they appear on the registrants birth certificate, passport or immigration papers
- I understand my child will be participating in an online learning program and that regular Internet service and a properly functioning computer either at home or arranged at the student’s school is needed
- I agree to have this enrollment and course work as part of the graduation plan for this student upon approval of an academic advisory or school counsellor
- I will ensure my child is keeping up with course demands, maintaining a good standing in their online courses and meeting course deadlines.
